If you would like to be seen by one of our physicians, you will need to obtain a referral from a doctor who is currently treating you (this is not an insurance based referral). We are a referral based practice only, we do not accept patients who are self-referring.

 

The current treating physician will need to provide all medical records, office notes, labs and testing associated with your confirmed diagnosis that we will be seeing you for. Upon receipt, our intake team will review and triage your medical case and we will reach out to you regarding appointments based on the urgency of your issue.

Our staff will not be able to schedule an appointment for you until all requested items are received and have been processed by our intake team.

 

If your current treating physician feels that you need to be seen for an urgent need, we request that the physician call our office directly to coordinate care.
