Questions

Do I need a referral to schedule an appointment?

YES. You will need to obtain a referral from a doctor who is currently treating you (this is not an insurance based referral). We are a referral based practice only, we do not accept patients who are self-referring.

 

The current treating physician will need to provide all medical records, office notes, labs and testing associated with the confirmed diagnosis that we will be seeing you for. Upon receipt, our intake team will review and triage your medical case and we will reach out to you regarding appointments based on the urgency of your issue. 

What are your office hours?

Our office hours are:

Monday - Friday from 8:30 am - 4:30 pm

The office will be closed daily, from 11:45 am - 1:00pm

What hospitals are your affiliated with?

All of our physicians are fully credentialid and privileged at Northwest Community Hospital in Arlington Heights and Amita Holy Family Medical Center in Des Plaines.

What types of disease do you treat?

We treat patients who have been diagnosed with (but not limited to): Bacterial infections, viral infections, HIV/AIDS. Blastomycosis, Complicated Urinary Tract Infections, MRSA, Pneumonia, Post-Operative Infections and Tuberculosis.

Do you take my insurance?

We accept most major private insurances including: Blue Cross Blue Shield, Aetna, Humana, Cigna, Anthem and United Healthcare among others.

We also accept Medicare and Medicare Advantage plans.

At this time, we do not accept Medicaid plans in office.

What do I need to bring to my appointment?

You will need to bring the following items to your first appointment:

Do you treat children?

No, we do not treat children.  We only treat patients that are 18 years of age or older. 

Will my Infectious Disease doctor prescribe pain medication for me?

Our providers do not  prescribe pain medicine or other controlled substances. Pain medications would need to be obtained from your primary care or other member of your care team.
